چکیده

BACKGROUND Clinical guidelines from the National Institute for Health and Clinical Excellence recommend the use of an intramedullary nail for the treatment of subtrochanteric fractures.1 Reverse oblique fractures are associated with varus malunion,2 which defunctions the abductors by placing them at a mechanical disadvantage and increases the lever arm across the fracture. Most modern nails have been designed with a lateral bend to accommodate a trochanteric entry point. This simple technique helps to avoid varus malreduction without the need for excessive traction or open reduction.

Intramedullary nailing of subtrochanteric fractures--does malreduction matter?

INTRODUCTION Subtrochanteric femur fractures remain challenging injuries to treat. Historically, varus malreduction has been linked to the development of nonunion; however, there is a paucity of literature evaluating the impact of sagittal plane malreduction. The purpose of this study was to evaluate the influence of coronal and sagittal plane malreductions on time to union of subtrochanteric f...

Burnei’s technique of femoral neck variation and valgisation by using the intramedullary rod in Osteogenesis imperfecta

BACKGROUND Varus or valgus deviations of the femoral neck in osteogenesis imperfecta have been an ignored chapter because the classic correction procedures were applied in medical practice with unsatisfying results. Until the use of telescopic rods, coronal deviations remained unsolved and the distal configuration of the proximal femoral extremity remained uncorrected or partially corrected, wh...

Failure of fixation of trochanteric femur fractures: Clinical recommendations for avoiding Z-effect and reverse Z-effect type complications

BACKGROUND Z-effect and reverse Z-effect are complications that arise from the surgical treatment of pertrochanteric fractures of the femur with proximal femoral nails (PFN) comprising two interlocking head screws. Such complications are induced by the migration of screws in opposite directions, which may lead to failure of the osteosynthesis. FINDINGS The paper describes three cases of pertr...

A Comparison of Internal Fixation and Bipolar Hemiarthroplasty for the Treatment of Reverse Oblique Intertrochanteric Femoral Fractures in Elderly Patients

PURPOSE To compare the clinical and radiological results between internal fixation using the proximal femoral nail system and bipolar hemiarthroplasty (BHA) in reverse oblique intertrochanteric hip fractures in elderly patients. MATERIALS AND METHODS From January 2005 to July 2012, we reviewed the medical records of 53 patients who had been treated surgically for reverse oblique intertrochant...
